The purpose of the form below is to inform the CCSAI of all details necessary for your event to be successful! Please be specific and detailed when completing the form. You have the option to save any information inputted and continue at a later time if you are not yet ready to submit the form, however you must complete the form within 30 days.

Club Event Proposal

When submitting your event proposals, please keep in mind the following lead times:
For virtual activities: Proposals must be submitted at least 2 weeks/10 business days prior to the proposed date of the activity
For hybrid and in-person activities: Proposals must be submitted at least 4 weeks/20 business days prior to the event
