Patio Picnic Registration

We’re excited to have you join us at the CCSAI Patio Picnic! Please complete this form to register for the event. The Patio Picnic features a variety of fun activities and amenities such as inflatables, BBQ and ice cream trucks, music by a live DJ, drinks, shirt and tote bag printing, balloon twisting, and face painting. This form also includes important photo consent and an inflatable activity waiver, so please review and acknowledge all sections before submitting.
