Club Executive Certificate Request Form

Complete the form below if you would like to receive a Certificate of Recognition for being a CCSAI Club Executive.

Upon requesting for your certificate, please allow up to 4 weeks for processing. You will be notified by the CCSAI when your certificate is ready. The deadline to request a certificate for the 2021/22 academic year is April 22, 2022.
